Apolipoprotein B (ApoB) is a structural protein found on all atherogenic lipoprotein particles, including LDL, VLDL, IDL, and Lp(a). Since each particle contains exactly one ApoB molecule, ApoB measurement provides a direct count of potentially atherogenic particles, making it a superior predictor of cardiovascular risk compared to LDL cholesterol alone. ApoB-100 is synthesized in the liver and serves as the primary structural protein for VLDL particles, which are subsequently metabolized to IDL and LDL particles. Each atherogenic particle carries one ApoB molecule throughout its lifespan, allowing ApoB to bind to arterial wall proteoglycans and initiate atherosclerosis.
