Red blood cell (RBC) magnesium measures the intracellular concentration of magnesium within erythrocytes, reflecting tissue magnesium stores more accurately than serum magnesium alone. Because approximately 99% of total body magnesium is intracellular, serum levels can appear normal even when cellular deficiency exists. RBC magnesium provides a more sensitive index of chronic magnesium status and is particularly useful when clinical symptoms suggest deficiency despite normal serum values. Magnesium is the fourth most abundant cation in the body and the second most prevalent intracellular cation, serving as a cofactor for over 300 enzymatic reactions including ATP synthesis, DNA replication, protein synthesis, and neuromuscular transmission. Magnesium is essential for hundreds of body functions including muscle and nerve activity, energy production, and blood sugar regulation. Most of the body's magnesium is stored inside cells, so a regular blood test can sometimes miss a deficiency. Low levels may be linked to symptoms like muscle cramps, fatigue, irregular heartbeat, or difficulty managing blood sugar. When elevated: Elevated RBC magnesium may indicate impaired renal clearance, excessive magnesium supplementation, or hemolysis artifactually elevating measured values. In the context of renal insufficiency, accumulation can contribute to neuromuscular depression, hypotension, and cardiac conduction abnormalities. Spurious elevation due to in vitro hemolysis is an important pre-analytical consideration. When low: Low RBC magnesium suggests chronic intracellular magnesium depletion and is associated with increased risk of cardiac arrhythmias, hypertension, insulin resistance, type 2 diabetes, migraine, muscle cramps, anxiety, and osteoporosis. It may also reflect poor dietary intake, malabsorption syndromes, or increased renal or gastrointestinal losses. Cellular deficiency can impair energy metabolism and exacerbate deficiencies of other electrolytes such as potassium and calcium.
