OmegaCheck is a proprietary cardiovascular risk assessment test developed by Cleveland HeartLab that measures the total omega-3 fatty acid content as a percentage of total fatty acids in red blood cell membranes, primarily reflecting levels of EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid). It is closely related to the Omega-3 Index and provides a standardized measure of long-chain omega-3 status over the preceding 8–12 weeks. The test is used to assess dietary omega-3 intake and its potential relationship to cardiovascular and inflammatory risk. OmegaCheck measures how much of the healthy omega-3 fats (EPA and DHA, found in fish and fish oil) are present in your red blood cells. This gives your doctor a picture of your omega-3 levels over the past 2–3 months. Higher levels are generally considered heart-healthy and are linked to lower risk of heart problems in research studies. Lower levels may suggest you are not getting enough omega-3s from your diet or supplements. Your doctor can use this result to discuss whether changes in your diet or supplementation might be beneficial for your heart health.

When elevated: Higher OmegaCheck values (≥8%) are generally considered favorable and are associated in observational studies with reduced risk of sudden cardiac death, lower triglyceride levels, and potentially reduced systemic inflammation. Very high values are not typically associated with adverse effects at physiologically achievable dietary levels, though extremely high supplemental doses may theoretically affect bleeding time. When low: Low OmegaCheck values (<4%) are associated with increased cardiovascular risk, higher rates of sudden cardiac death in epidemiological studies, elevated triglycerides, and a pro-inflammatory physiological state. Low values may also be associated with suboptimal cognitive and mood-related outcomes in some research contexts, though causality has not been firmly established.
