The QuestAssureD panel is a comprehensive vitamin D assessment that simultaneously measures 25-hydroxyvitamin D (25-OH D, the primary storage and circulating form) and 1,25-dihydroxyvitamin D (calcitriol, the biologically active hormonal form). 25-OH D reflects overall vitamin D status and nutritional sufficiency, while 1,25-dihydroxyvitamin D reflects renal activation and end-organ hormonal activity. Together, these two metabolites provide a more complete picture of vitamin D metabolism than either marker alone. Vitamin D undergoes sequential hydroxylation: first in the liver to 25-OH D (calcidiol), then in the kidneys (and some extrarenal tissues) to 1,25-dihydroxyvitamin D (calcitriol) under tight regulation by parathyroid hormone (PTH), fibroblast growth factor-23 (FGF-23), and serum calcium and phosphate levels. Calcitriol acts on nuclear vitamin D receptors (VDR) throughout the body to regulate calcium and phosphate absorption, bone remodeling, immune function, and cell differentiation. Because renal conversion is tightly regulated, 1,25-dihydroxyvitamin D levels do not reliably reflect overall vitamin D stores and can be normal or even elevated in the setting of significant 25-OH D deficiency.

Having both results together helps your doctor understand not just whether you have enough vitamin D, but also whether your body is processing it correctly. Abnormal results in either direction can have health implications and should be discussed with your healthcare provider.

When elevated: Elevated 25-OH D may indicate vitamin D toxicity or excessive supplementation, potentially leading to hypercalcemia and soft tissue calcification. Elevated 1,25-dihydroxyvitamin D can indicate ectopic calcitriol production (as seen in granulomatous diseases such as sarcoidosis, tuberculosis, or lymphoma), primary hyperparathyroidism, or certain vitamin D-dependent conditions; this can cause hypercalcemia and hypercalciuria independent of 25-OH D levels. When low: Low 25-OH D indicates insufficient vitamin D stores, associated with increased risk of metabolic bone disease, secondary hyperparathyroidism, muscle weakness, and impaired immune function. Low 1,25-dihydroxyvitamin D in the context of adequate 25-OH D suggests impaired renal activation, as seen in chronic kidney disease, hypoparathyroidism, or pseudohypoparathyroidism, and may contribute to hypocalcemia and renal osteodystrophy.
