Thyroglobulin Antibody

Thyroglobulin antibody (TgAb) is an autoantibody directed against thyroglobulin, the large glycoprotein precursor to thyroid hormones T3 and T4 synthesized within the thyroid follicles. Elevated TgAb is a hallmark of autoimmune thyroid disease, most notably Hashimoto's thyroiditis and Graves' disease, and its detection carries diagnostic and prognostic significance. In patients with differentiated thyroid cancer, TgAb is critically important because it interferes with serum thyroglobulin measurement, the primary tumor marker used for surveillance. Thyroglobulin is produced exclusively by thyroid follicular cells and serves as the scaffold upon which iodination and coupling reactions generate thyroid hormones stored in the follicular colloid. Under normal immune tolerance, thyroglobulin is sequestered from systemic immune surveillance; when this tolerance breaks down—due to genetic predisposition, environmental triggers, or thyroid injury—B lymphocytes generate IgG autoantibodies against multiple thyroglobulin epitopes. These antibodies may contribute to complement-mediated and cell-mediated thyroid destruction, though their precise pathogenic role versus bystander status remains an area of active investigation.

When elevated: Elevated TgAb strongly suggests autoimmune thyroid disease and warrants evaluation for Hashimoto's thyroiditis or Graves' disease, including TSH, free T4, and anti-TPO antibody testing. In patients with differentiated thyroid cancer, elevated TgAb renders serum thyroglobulin measurements unreliable (typically falsely low), necessitating use of TgAb trend as a surrogate tumor marker and consideration of imaging for surveillance. Persistently high TgAb in euthyroid individuals is associated with a 2–4% annual risk of progression to overt hypothyroidism and may also be associated with adverse pregnancy outcomes including miscarriage and postpartum thyroiditis. When low: Undetectable or very low TgAb is generally reassuring and indicates absence of significant autoimmune thyroid activity at the time of testing. In thyroid cancer surveillance, a negative TgAb result validates the reliability of serum thyroglobulin as a tumor marker, improving the sensitivity of cancer recurrence detection. Low TgAb has no established adverse clinical implications in isolation.
