Free thyroxine (FT4) is the unbound, biologically active fraction of the thyroid hormone thyroxine (T4), measured directly without prior separation steps. Unlike total T4, FT4 is not affected by changes in thyroid-binding globulin (TBG) concentrations, making it a more reliable indicator of true thyroid hormone status. It is a cornerstone test in the evaluation of thyroid function, used alongside TSH to characterize hypothyroid and hyperthyroid states. T4 is synthesized and secreted by the thyroid gland under stimulation by pituitary TSH, with approximately 99.97% of circulating T4 bound to carrier proteins such as TBG, transthyretin, and albumin. Only the free fraction (~0.03%) is metabolically active, entering target cells where it is converted to the more potent triiodothyronine (T3) by deiodinase enzymes. FT4 participates in a classic negative feedback loop with the hypothalamic-pituitary axis, suppressing TRH and TSH secretion when levels are adequate.
