Zinc, RBC

Red blood cell (RBC) zinc measures the intracellular zinc concentration within erythrocytes, reflecting zinc status over the preceding 2–3 months corresponding to erythrocyte lifespan. Unlike serum zinc, which is subject to acute-phase fluctuations and diurnal variation, RBC zinc provides a more stable index of longer-term zinc nutritional status. It is considered a more sensitive and reliable biomarker of zinc sufficiency than serum or plasma zinc alone. Zinc is an essential trace element serving as a catalytic cofactor for over 300 enzymes and more than 1,000 transcription factors, playing critical roles in DNA synthesis, immune function, protein metabolism, and antioxidant defense. Within erythrocytes, zinc is predominantly bound to carbonic anhydrase and superoxide dismutase (Cu/Zn-SOD), making RBC zinc a functional indicator of these enzymatic activities. Zinc homeostasis is regulated through intestinal absorption, hepatic storage, and renal excretion, with the body having limited storage capacity, making regular dietary intake essential.

What this test reveals

expand_more

This test measures the amount of zinc inside your red blood cells, which gives a picture of your zinc levels over the past several weeks to months. Zinc is a vital mineral your body needs for a healthy immune system, wound healing, normal growth, and many other functions. Low levels may suggest you are not getting or absorbing enough zinc from your diet, while high levels are often related to taking too much zinc in supplements. Your doctor will interpret this result alongside your symptoms, diet, and other lab tests to determine if any action is needed.

warning

What abnormal values may indicate

expand_more

When elevated: Elevated RBC zinc may indicate excessive zinc supplementation or intake, which can competitively inhibit copper absorption and lead to secondary copper deficiency. Persistently high levels warrant review of supplement use and dietary sources of zinc. When low: Low RBC zinc suggests suboptimal or deficient zinc status over a prolonged period, which may be associated with impaired immune function, poor wound healing, growth failure in children, hypogeusia (reduced taste acuity), dermatologic changes, and increased susceptibility to infection.

exercise

For athletes

expand_more

RBC zinc reflects sustained zinc status over 2–3 months, relevant to athletes because intensive training increases oxidative stress and immune demand, both zinc-dependent processes. Adequate RBC zinc supports recovery, maintains immune resilience during high training load, and sustains the antioxidant enzymes (SOD) that protect muscle tissue—making it a useful long-term marker for training readiness and durability.
