Total PSA is the standard screen, the free-to-total ratio helps interpret a borderline result, and ultrasensitive assays are used to follow very low values after treatment. Men order these to establish a baseline in their forties, to track a rising trend year over year, or because a family history moves screening earlier. A PSA result is a conversation starter with a clinician, never a diagnosis on its own.
